This information will explain why Life Balance Counselling asks for your personal data, how it is stored and used, who I may share it with and what your rights are.
Why I ask for your personal information.
Any personal information that the controller, Hilary Sims, will ask for either via email or in person, eg. name, address, telephone number, email, emergency contact and any known medical problems is only for the purpose of keeping in touch with you and for your own safety during the sessions. The emergency contact details are kept in case an ambulance had to be called whilst at Life Balance Counselling.
Who is your data shared with.
Your personal data is not shared with anyone else who less I have to break confidentiality as per UK regulations.
How your data is stored.
Your data is stored manually in a locked filing cabinet.
How long is your information kept for?
Your information will be destroyed 7 years after our last appointment together (as per British Association for Counselling and Psychotherapy guidelines)
You have the right to see information held by Life Balance Counselling by submitting a subject access request in writing to Hilary Sims, 8 Colshaw Road, Stourbridge, West Midlands, DY8 3AS. You can opt out of this and ask for your details to be deleted at anytime by submitting a request in writing.
